JACK DAVID FORMAN was born to Edward Frederick and Olive (Moriarty) Forman on January 17, 1938 in Pipestone, Minnesota where he was raised and attended school. After his graduation from Pipestone High School in 1957, he worked for Bayliner in Pipestone until he moved to Los Angeles, California. A few years later he returned to Pipestone. In August of 1970, he moved to Willmar, Minnesota where he worked at the Lakeland Hotel and The Chinese Restaurant. He was also employed as a custodian for the Willmar Housing and Redevelopment Authority. After he retired in 2001, he worked part-time for Vince Van Heuvelen as a night watchman. On March 1, 2017, Jack passed away at the Rice Memorial Hospital in Willmar at the age of 79 years, one month, and 14 days.
Jack was a member of St. Mary’s Catholic Church in Willmar, Minnesota. When talking of his faith he stated, “You shouldn’t think of how many reasons you don’t want to go to church, but rather think of how many reasons you should go to church.” He was a history buff, and he had a vast knowledge of the Civil War and also World War II. Jack lived his life independently, asking nothing from anyone and was proud of his independence.
He is lovingly remembered by his sister, Sharon (Robert) Streetar of Cohasset, Minnesota; three nephews, Robert (Jean) Streetar, Michael (Pamela) Streetar and their children, Sam, Cadi, and Kelli, and Tom (Michelle) Streetar and their children, Max, Olivia, and Owen. He was preceded in death by his parents; and his sister, JoAnn Forman.
